What is Needed For My First Visit?

1-Complete the Intake Form: Please fill out the intake form by pressing one of the buttons below. We will reach out to you with what we need for your first visit.

2-Arrive Early: Plan to arrive 5-10 minutes before your appointment to allow time for sign-in and an overview of what to expect during your visit.

3-A Parent or Caregiver Must Be Present: During evaluations, we require that a parent or caregiver accompanies the child. For everyone’s comfort, we allow a maximum of two adults per visit.
